Workshop facilitators

Advance Care Planning Australia have developed a range of education packages and materials to assist educators to facilitate face-to-face workshops with health and care workers in:

  • Advance care planning - key principles and practice
  • Advance care planning in primary care
  • Advance care planning in the context of dementia.

These materials have been designed for use nationally and are provided free-of-charge to facilitators managing their workshops on this website.

What's required?

It is recommended that facilitators delivering advance care planning workshops have the following:
  • Experience delivering training in a health care or health service environment
  • Have viewed online education modules available on the Advance Care Planning Australia Website
  • Be familiar with initiating advance care planning conversations
  • Be familiar with the specific state legislative requirements

How to get started

Simply submit your information and workshop details to receive access to facilitator materials and to have your workshop publicised on Advance Care Planning Australia Learning.

  • Having your workshop featured on this website under 'Upcoming Workshops'
  • State/Territory specific website links, downloadable forms and other useful resources
