Health professionals and aged care workers aim to provide care that is patient-centred and delivered in line with a person’s goals, beliefs and preferences, particularly as people age and become more likely to have multiple chronic conditions.

Decision Assist is delivering a series of free workshops in South Australia, in partnership with Adelaide PHN and Country SA PHN. Workshops will improve knowledge, skills and confidence in advance care conversations.

They will also explore ways to better identify and provide palliative care to people in a timely and effective way.

Topics covered include:

  •      What are advance care directives (ACDs)?
  •      What are your roles and responsibilities?
  •     How do you communicate effectively about medical treatments, ACDs and end-of-life care?
  •    How do you support your patient through the ageing process and palliative care?

All health care professionals from general practice, aged care, allied health and community agencies are invited to attend these interactive sessions.
